Use these forms to enroll your student. (Both forms are needed for prospective families intending to enroll a student).

Student Photo Permission Form

Use this form to grant permission to Cornerstone Christian School to post pictures which include your student and/or to post school works/projects/art works created by your student on the school website.

Student Dismissal Form

Use these form to authorize someone (other than yourself) to pick-up/carpool your student.
